WAIVER, CONSENT, RELEASE OF LIABILITY, AND AGE & GRADE VERIFICATION Midwest Lacrosse Club 1A. COVID-19 NOTICE COVID-19, a global pandemic declared by the World Health Organization, is highly contagious and primarily spreads through person-to-person contact. While the Midwest Lacrosse Club (“MIDWEST”) follows best practices to reduce exposure risk, we cannot guarantee that you or your child(ren) will not contract COVID-19 by attending our events or programs. By signing this waiver, I acknowledge the risk of COVID-19 and voluntarily accept the possibility that my child(ren) and I may be exposed to or infected by the virus while participating in MIDWEST activities. I understand such exposure may lead to personal injury, illness, disability, or death. I further confirm that my child(ren) is in good health, symptom-free (including no fever, cough, sore throat, shortness of breath, body aches, or loss of taste/smell for the past 48 hours), and has not knowingly been in contact with anyone with COVID-19 symptoms or recent international travel in the past 14 days. I accept all associated risks and agree to release and hold harmless MIDWEST, its staff, volunteers, participants, and representatives from any claims or liabilities related to COVID-19 exposure during or after MIDWEST events. WAIVER AND RELEASE Participation in lacrosse involves inherent risks, including serious injury, permanent disability, or death. I understand that such risks may arise from my actions or those of others, including negligence. I accept full responsibility for my safety and that of my child(ren) during participation in all MIDWEST events. I voluntarily assume all risks and release MIDWEST, its affiliates, coaches, organizers, volunteers, and staff from any liability for injury, damage, loss, or expense related to my child(ren)’s participation in any lacrosse event, including risks related to equipment use or exposure to infectious diseases. CONSENT FOR MEDIA USE I grant permission to Midwest Lacrosse Club to use photos and/or videos of my child(ren) for promotional purposes in online and printed media. This includes, but is not limited to, websites, social media, advertising materials, and publications. MEDICAL EMERGENCY CONSENT In the event of a medical emergency, I authorize Midwest Lacrosse Club and its representatives to provide or seek appropriate medical treatment, including first aid, hospitalization, or emergency care for my child(ren). I understand that every reasonable effort will be made to contact me or another legal guardian before action is taken. I accept financial responsibility for any medical costs incurred, acknowledging that MIDWEST does not carry health insurance for participants. AGE & GRADE VERIFICATION For safety and fairness, accurate age and grade information is mandatory. I certify that the grade and age provided for my child(ren) is correct. I understand that falsifying this information may lead to disqualification of my child and team from all MIDWEST events. I also agree to provide verification such as a school report card if requested. FINAL ACKNOWLEDGEMENT I HAVE READ AND UNDERSTOOD THIS WAIVER, CONSENT, AND RELEASE OF LIABILITY. I AM SIGNING VOLUNTARILY AND CERTIFY THAT I AM A PARENT OR LEGAL GUARDIAN OF THE PARTICIPATING ATHLETE. I AGREE TO BE BOUND BY THE TERMS AND CONDITIONS HEREIN ON BEHALF OF MY CHILD(REN).
